Student Athlete Information Forms
Before competing in intercollegiate athletics at Campbellsville University, each prospective student-athlete must complete their respective medical clearance forms and be approved for participation by the Campbellsville University Sports Medicine staff.
     

Please be sure that you are filling out each form completely and accurately. DO NOT FORGET, a copy of your current primary insurance card is needed! Failure to complete all forms may result in the delay of your participation in Tiger or Lady Tiger Athletics.
